The theme for this year’s Healthcare Estates, is Mind the Gap – are we heading for a two-tier estate.
After more than 75 years working alongside NHS Trusts, one lesson stands above all others: buildings do not fail suddenly; they decline through thousands of deferred decisions. The concern today is not simply an ageing estate, but the prospect of a two-tier one—where well-funded sites benefit from resilient infrastructure, low-carbon systems and modern clinical environments, while others remain dependent on fragile plant, temporary fixes and rising maintenance costs. The gap is increasingly visible. Some buildings have heat pumps, intelligent controls, on-site generation and coordinated replacement programmes. Others have obsolete boilers, switchgear and ventilation systems operational beyond their intended lives.
Every patient and member of staff should reasonably expect safe temperatures, dependable power, suitable ventilation and infrastructure capable of supporting modern care. When backlog maintenance begins to shape clinical choices, impede decarbonisation or increase avoidable downtime, the estate is no longer a background service, it becomes a strategic risk.
